Some employees may choose to purchase supplemental life insurance coverage. if you’re eligible, you can buy coverage for yourself. and for managers, if you purchase this coverage, you may choose to purchase life coverage for your spouse/domestic partner and eligible dependent children. employees pay the full cost of this coverage.

If you qualify, you can purchase supplemental life insurance coverage in increments of $10,000, up to a maximum benefit of $300,000. The coverage you purchase cannot exceed three times your base salary. all managers, all unions can buy up to five times their base salary up to a maximum of 1,000,000.
Premiums are based on your age and the level of coverage you select. you pay the full cost of this coverage. see plan costs for current premiums.
You must designate one or more beneficiaries for the supplemental life insurance benefit. (A beneficiary is the person who receives the benefit in the event of your death.) You can add or change a beneficiary by completing the Beneficiary Designation Change Form and returning it to the Employee Benefits Center (EBC). You can make changes to your beneficiary designation at any time.

If you purchase supplemental life insurance coverage for yourself, you may purchase coverage in $10,000 increments for your eligible spouse/domestic partner. you can elect up to $150,000 in coverage; however, the coverage you purchase for your spouse/domestic partner cannot exceed the amount of your supplemental life coverage.
Premiums are based on the age of your spouse or domestic partner and the level of coverage you select. you pay the full cost of this coverage. see plan costs for current premiums.
You are automatically the beneficiary of this coverage.

If you elect supplemental life insurance coverage for yourself, you may choose to purchase dependent children’s life insurance for your eligible dependent children in one of the following amounts:
- $2,500
- $5,000
- $7,500
- $10,000
Premium costs vary only depending on the option you choose – you pay the premium, regardless of the number of children you choose to cover. you pay the full cost of this coverage. see plan costs for current premiums.

You pay the premiums for supplemental life insurance coverage. Employee and spouse/domestic partner life insurance costs are based on the age of the covered person and the level of coverage chosen. Coverage costs for dependent children are based solely on the level of coverage you select.
